Patient Paperwork
In our office, we utilize online/digital intake paperwork. After your appointment is scheduled, you'll receive an email with a private link containing all necessary paperwork.
These documents can be completed and submitted prior to your first visit, allowing us to save you time and prepare appropriately for your initial appointment.
If you can not fill out your new patient paperwork ahead of time, please arrive 15 minutes early for your first appointment.

What if I don't have insurance?That's no problem. We have affordable treatment options for anyone who does not have chiropractic coverage. We’ll be happy to discuss those options with you!

What should I expect on my first visit?Please bring both your ID and your insurance card (if you have one) to the first visit. We will ask you to fill out some paperwork to give us insight into your symptoms and your medical history. After a thorough examination and any necessary imaging, our Chiropractors will determine whether chiropractic care is a good fit for you. You may also receive treatment on the first visit depending on your specific case.

While we believe it’s a great option most of the time, we’re also happy to give you other options and referrals if we determine that’s a better option.
